Please print out this form, fill it in, and mail it to us with your check made out to CorgiAid for the total amount. The address is on the form. If you don't have a printer handy, copy off the needed information and send a letter and check.

If you'd like to write your own letter, please tell us the number of copies you'd like. If your order is for less than five books, the price is $30.00 and $8.00 shipping and handling per set. Please make your check out to CorgiAid, Inc., and mail your order to:
